Please submit your paperwork through the Athletic Clearance Website. If you need any technical help please contact Coach Dunn (386-822-6909 Extension 23300 or via email). Athletic forms can be picked up at the school if you're unable to print the forms below. The campus is open Monday - Thursday: 9am-4pm.
School Based Insurance for Athletes (All Athletes must have insurance. This is an option for student athletes who do not have insurance with their parent/guardian).

Pay to Participate Form (due by first regular season contest date)
